This might be a regression since there was a post in March 2026 that was similar.
When a user sets the option to prepend [SPAM] to the subject it writes it into all three policy fields, spam_subject_tag, spam_subject_tag2, and spam_subject_tag3.

I have to run the command:
mysql amavisd -e "UPDATE policy SET spam_subject_tag=NULL;"

every few minutes on cron to make sure it is caught and returned to null.

I am not sure when this started, I only have one user that uses this feature so was not aware of the issue till now.
