I haven't set up /etc/hosts to be anything different than the default so I'm not sure what that should be changed to. Any help on what I can do?

This is normal. DKIM and SPF help prevent this issue, but it's not sufficient.

What else do I have to do in order to get around it? I can't even send mail to Hotmail because it think my emails are spam.

If hotmail rejects your email, it will return some useful info and your Postfix will log it. So the question is: What's the error message in your Postfix log file when you send email to hotmail?

You don't know how recipient (gmail) detects spam, so it's very hard to avoid this.

With Hotmail you better register yourself at https://sender.office.com

Also you might want to use Yahoo Bulk Sender form.

As for Gmail...  Well,  you need whatever they want.  Read:
http://mail.google.com/support/bin/answ … swer=81126

Other than these you need to have a clean IP and SPF,  DKIM, Reverse DNS, DMARC records and maybe DomainKeys in order to not be eliminated in their first checks.

Also the content of your email does affect where your mail show up as well.  If you write spammer crap,  you'll end up at Spam folder.  Try checking the header information and check your SpamAssassin score.

Also you may try tools like http://mail-tester.com and http://allaboutspam.com by sending them an email and checking the result.

Lastly use MXToolBox and sites like http://emailsecuritygrader.com to review your setup.

Even then it's never guaranteed (:
