A previous question I had was about determining if the /var/spamassassin config files were actually being used.  That issue was resolved and I was able to determine they indeed were being used.

Turns out, SpamAssassin appear to be working.  The problem is that the email going to the /Maildir/.Junk folder does appear to be scanned and marked as spam, but the email in the /Maildir/new folder is not being marked.

How can I make sure this email is scanned and properly marked?

1: SpamAssassin can not identify EVERY spam emails. so it's normal that some emails went to INBOX folder.
2: You can check Amavisd log file to see the spam scanning result of each email.

Apparently I made a mistake when I was checking to see if messages appear in the Maildir/new and Maildir/.Junk/new folders. 

I understand #2, and I've been working to tune the configuration to catch more of these emails. 

Where in the config is setting for the incoming email marked as spam to go to the Maildir/.Junk/new folder?

Check /var/vmail/sieve/dovecot.sieve.
