Please install port 'www/mod_auth_mysql_another' and try again.

Find Apache config file under /usr/local/etc/apache22/, and enable module 'mysql_auth_module'.
Just look like below:

LoadModule mysql_auth_module libexec/apache2/mod_auth_mysql.so
